You may nominate yourself or others. There is no limit to the number of nominations you may submit. Please complete a separate form for each nomination.

All shortlisted nominees and their nominators will receive complimentary tickets to the Celebration and Awards Night on the 11 June 2026, in Depot, Cardiff.
